Person-centered monitoring, training, education, demonstration, or support to assist the individual with the acquisition and retention skills in the following areas:
Leisure activities and community/public events.

• Educational activities
• Hobbies
• Unpaid work experience (ex. Volunteer opportunities)
• Maintaining contact with family and friends

Training and education in self-direction designed to help individuals achieve one or more of the following outcomes:

• Develop self-advocacy skills
• Exercise civil rights
• Acquire skills that enable self-control and responsibility for services and supports received or needed
• Acquire skills the individual to become more independent, integrated, or productive in the community.

Available under Community Integration and Habilitation waiver

• Assistance with acquiring, enhancing, and building natural supports.
• Working with the individuals to meet the goals they have set for themselves.
• Training the individual to enhance their homemaking skills, meal preparation, household chores, money management, shopping, communication skills, and positive behavior.
• Provision of transportation to fully participate in social and recreational activities in the community.
• Coordination and facilitation of medical and wellness services to meet the healthcare and wellness needs, including physical consults, medication, implementation of risk plans, dining plans and wellness plans.
• Maintance of individuals health records.

Living arrangement in which an individual lives in the private home of a principal caregiver who may be a non-family member (foster Care) or a family members who is not the individual’s spouse, the parent of the individual who is a minor, or legal guardian of the individual.

Available under Family Support Waiver and Community Integration and Habilitation waiver.

Respite services are provided for individuals unable to care for themselves thats are furnished on a short-term basis because of the absence or need for relief of those personas who normally provide care for the individual. Respite services can be provided in an individuals home, respite providers residence, or residential setting.
